Nose Aesthetics (Rhinoplasty) is an important surgical procedure that not only addresses aesthetic concerns but also offers solutions to breathing problems. Since each patient’s facial structure, skin type, and nasal anatomy are different, this procedure requires a personalized approach and, when performed by the right hands, provides satisfactory results both aesthetically and functionally. In the Rhinoplasty surgeries we perform at our clinic, the most appropriate technique is selected by taking into account the patient’s expectations and anatomical structure, and a personalized treatment plan is created.
Surgical Techniques in Nose Aesthetics
Nose aesthetic surgeries are basically performed using two main techniques: the open technique and the closed technique. In open Rhinoplasty surgery, the operation is performed by making a small incision along the lower line of the nose, whereas in closed Rhinoplasty surgery, the entire procedure is carried out from inside the nose without making any external incisions. The success rates of both methods are similar when applied correctly and appropriately; however, the choice of technique depends on the patient’s anatomy and the issues that need to be corrected.
In my clinical practice, I particularly prefer the open technique for severely deviated noses and revision (previously operated) nose surgeries. Prior to these operations, a detailed analysis is performed for the patient, studies are conducted on photographs, and the patient is informed about the expected outcome through Photoshop drawings or simulations. If the patient requests, multiple consultations can be held before the surgery to answer all questions and ensure the patient is fully informed.
Thanks to the total subperichondrial technique we use during surgeries, a significant reduction in postoperative swelling and bruising is achieved. In addition, we aim to minimize edema by continuously applying cold water and ice during the operation. Bone modifications and cutting procedures are performed using micro motors or micro saws, thereby minimizing tissue damage. Instead of weakening the nasal skeletal structure, shaping is performed by preserving the cartilage and bone structures. In this way, we prevent long-term structural deterioration of the nose and the occurrence of nasal drooping.
Rhinoplasty Procedure and Anesthesia Options
Aesthetic Nose Surgeries can be performed under general or local anesthesia depending on the patient’s condition and preference; however, general anesthesia is mostly preferred. Although the duration of surgery varies from person to person, it takes an average of 1.5–2 hours. During this time, we do not focus solely on the external appearance of the nose; deviations inside the nose, enlarged nasal turbinates (inferior turbinate hypertrophy), concha bullosa (ballooning of the intranasal turbinates), and sinus problems such as sinusitis can also be corrected.
One of the most important differences of our clinic is that we prioritize patient comfort after surgery. Instead of traditional nasal packs that cause discomfort, we use perforated silicone splints. Thanks to this, our patients can breathe comfortably even immediately after surgery. These splints are usually removed 2–4 days after surgery, while the splint and tapes on the nose are removed within 8–12 days.
The postoperative recovery period is also of great importance. Although edema and mild bruising are normal in the first days, thanks to the special techniques we use, bruising remains minimal or does not occur at all in most of our patients. When the splint is removed, the nose still appears swollen and has not yet taken its final shape; however, as the swelling subsides over time, the shape becomes clearer. The time required for a nose to take its final shape may vary between 6 months and 2 years depending on the patient’s skin type and many other factors.
Preoperative Preparation and Recovery Process in Rhinoplasty
Before every rhinoplasty surgery, we conduct a comprehensive preliminary evaluation with our patient. During the first consultation, detailed photographs are taken, the patient’s expectations are listened to, and simulation studies are performed. In light of this information, a personalized surgical plan is created. When deemed necessary or upon the patient’s request, multiple consultations can be carried out before the surgery.
During the postoperative period after Rhinoplasty Surgery, we offer our patients a personalized care program. Swelling and bruising usually decrease significantly within the first week. After the silicone splints are removed 2–4 days later, the patient begins to breathe more comfortably. Once the splint and tapes on the nose are removed after 8–12 days, an idea about the general appearance of the nose can be obtained; however, at this stage, the nose is still swollen and has not yet taken its final shape.
By the end of the first month, as the swelling decreases significantly, the nose gains a clearer appearance; however, patience is required for fine details to become prominent and for the nose to completely take its final shape. During this period, the healing process is closely monitored through regular follow-up appointments, and interventions are made when necessary. We provide our patients with detailed information about points to pay attention to, activities to avoid, and methods to protect the nose during this process.
